The tale of woe of "Madame Julie", Carthusian mix, approx. 10 years old
- Accident with dog on 16.12. - cat was bitten, thrown around and then crawled away, could be caught on 19.12. and brought to the University Hospital Berlin, double jaw fracture at the bottom, several bite injuries on the abdomen & back, laceration in the groin
- was after stabilization and drip still on 19.12. emergency-operated, the suspicion of an injury to the abdominal cavity was not confirmed.
- Course good, blood values satisfactory, use of blanket ABs, painkillers, etc. Parts of the wound suture "do not like" the treating surgeon and he "wants to observe", discharge planned for 27.12., medication slowly reduced towards the end
- 26.12. spontaneous change of plan, another operation, inflammation values extremely high, massive pus accumulation at wound opening, wound irrigation and renewed closure of the wound, painkillers, general ABs
- increasingly worsened general condition over the next few days, new operation planned by the professor of the university hospital, blood donor search was advised because hematocrit already below 20%, on 31.12. new operation, massive fat necrosis, removal of necrotic tissue, wound irrigation, blood donation via donor cat, painkillers and general ABs, gastric tube placed, VAC wound system installed, dressing changes under anesthesia every 2 days
- initially restrained improvement of condition, hematocrit at 24%, feeding via tube, reduced granulation with conspicuous amount of exudate, primary wound closure after 3 VAC changes despite unsatisfactory course
- Condition worsens increasingly, wound edges do not adapt and die partially, clear symptoms of cat rhinitis, pus flakes in the still abnormally much wound water, nutrition only via tube, because of hardly defecation lactulose is given, during the night feces runs into the dressing, infection with coli germs, at the same time an antibiogram shows that the present germ is/was resistant to the antibiotic used so far, adaptation of the latter, then slight improvement of the general condition
- Wound suture bursts again on 14.01., renewed general anesthesia with cleaning of the wound suture, renewed installation of a VAC bandage, further significant deterioration of the general condition, hematocrit at 16%, suggestion of the clinic: renewed surgery with removal of a skin flap from the knee area to close the wound, since skin is now slowly missing, secondary wound healing practically excluded according to the collective opinion of the doctors.
- Proposal of the clinic rejected, 2 new blood donations organized within 3 days, discontinuation of all medications, removal of the stomach tube and preparation for a secondary wound healing, pick up of the cat on 21.01. with hematocrit at 19%
- due to the many anesthesias, the fasting associated with it (clear protein deficiency visible in BB), medications and almost daily blood sampling cat very dull, hardly any color in the gums, wound poorly perfused, skin hanging conspicuously, tissue is sluggish, wound healing non-existent
- at home for the time being only care, feeding 4 times a day 40-50ml each reconvales, pureed cat food, vitamin B12 & D extra, 3 times a day 2 ml colloidal silver orally, calendula C30, dressing change with poly sponge overlay every 2 days, rinsing and permanent moistening with colloidal silver 25 ppm
- on January 30 in clearly better perfused wound small spots of granulation tissue visible for the first time, wound edges start to bleed, moderate wound odor
- on 31. January cat in stable condition but still clearly reduced and still hand fed brought to Claudia Susann Buchholz, there from 04.02. at least 1x daily treatment of the wound area with the cold plasma of ASmax directly on the wound or applied through the dressing
